SPECIALTY CARE PLANS · DISCHARGE TRANSITION
Hospital-to-Home Care Plan
A hospital-to-home care plan is non-medical support for clients recently discharged from a hospital or rehab facility, built around a safer, calmer transition.
The days right after discharge carry the highest risk of a repeat hospital visit. This plan is designed specifically around those first, critical days home.
IMPORTANT: NON-MEDICAL SUPPORT
Golden Rule Home Care provides non-medical support only. We do not diagnose, treat, or manage medical conditions, and we do not replace a physician, nurse, or emergency provider. Our caregivers can help with medication reminders, watch for changes or symptoms that matter, and report them promptly to your family and, when appropriate, your loved one’s medical team.

Can care start immediately after discharge?
Yes, we work to coordinate care to begin as soon as a client arrives home, and can often accommodate short-notice discharge dates with advance notice from the family or hospital.
