Stroke Support Care Plan

A stroke support care plan is patient, non-medical in-home care for clients recovering from stroke or living with stroke-related changes.

Every recovery timeline looks different. This plan meets your loved one where they are, rebuilding confidence, routine, and independence at their own pace.

IMPORTANT: NON-MEDICAL SUPPORT

Golden Rule Home Care provides non-medical support only. We do not diagnose, treat, or manage medical conditions, and we do not replace a physician, nurse, or emergency provider. Our caregivers can help with medication reminders, watch for changes or symptoms that matter, and report them promptly to your family and, when appropriate, your loved one’s medical team.
